Customer Proprietary Network Information Notice

In the course of doing business with Integra Telecom and its operating affiliates listed below, we will have access to your telecommunications service information, which includes the types of services you purchase, how you use them, and the related billing of those services. This is called customer proprietary network information or CPNI. You have the right, and we have the duty, under federal law, to protect your CPNI.

Integra has never provided and will never provide your CPNI with any third party that is not authorized to access your CPNI. In order to serve you better, we may use your CPNI, without further authorization by you, to inform you of new features for existing services you may have, as well as the full range of communications-related products and services available from Integra Telecom. Without further authorization by you, we may share your CPNI with the Integra family of companies and affiliates who offer communications-related services. These companies currently are Integra Telecom of Washington, Integra Telecom of Oregon, Integra Telecom of Minnesota, Integra Telecom of North Dakota, Integra Telecom of Utah, and Electric Lightwave, LLC.

No action by you is necessary to permit us to use your CPNI as described in this notice. You have the right to disapprove the uses of CPNI described herein. You can deny or withdraw access to your CPNI at any time. If you wish to restrict Integra Telecom's and its affiliates' use of your CPNI to offer services different from the types of services you currently buy from us, please register your request by visiting http://cpni.integratelecom.com within thirty (30) days of receipt of this notice. (We will not use your CPNI prior to this time, except as required or permitted by law.) You may change your decision at any time and your decision will remain valid until you tell us otherwise. Any approval or denial of approval for the use of CPNI outside of the service(s) to which you already subscribe is valid until you affirmatively revoke or limit your approval or denial. Your service will not be affected by this decision.
